SUMMARY OF BENEFITS

 

INSURANCE (HEALTH & DENTAL)

Insurance is effective on the first day of the month following three months of employment.  Amteck pays 90% of the single portion of Health insurance; dental insurance is paid 100% by the employee through pre-tax payroll deductions.  You will receive an enrollment packet during the 2nd week of the month in which you complete your three month waiting period.

These policies renew every August 1; Amteck reserves the right to modify these benefits at this time.

SHORT TERM DISABILITY

Disability insurance is available following the three month waiting period.  The employee pays 100% of the premium, and there is no open enrollment for this benefit.  You must sign up during your New Hire Enrollment period.

LIFE INSURANCE

Amteck provides a $20,000 Life Insurance policy, as well as a $20,000 Accidental Death & Dismemberment policy, at no cost to the employee.  Each employee receives this benefit following the completion of their three month waiting period. Additional Supplemental Life insurance policies are available at the request of the employee (to be paid 100% by the employee).

401K RETIREMENT PLAN

Participation is available following ONE FULL YEAR of employment and completion of at least 1,000 hours, with quarterly enrollments (January 1, April 1, July 1, October 1).  You will receive an enrollment packet prior to your eligible quarterly enrollment date. Amteck matches 50% up to 6%.  Contributions are made on a pre-tax basis.

FLEXIBLE SPENDING ACCOUNT – MEDICAL AND/OR DEPENDENT CARE

Participation is available following ONE FULL YEAR of employment.  You will receive an enrollment packet at the first of the month prior to your one year anniversary month.

HOLIDAYS

Employees are eligible for Holiday pay following completion of three months. You must work the scheduled day before and after the holiday.  You will receive Holiday pay if you scheduled the day before or after off prior to that date (Human Resources must have a Time Off Request form from you prior to that date).  Recognized Holidays are:

  • Memorial Day
  • Independence Day (July 4th)
  • Labor Day

  • Thanksgiving Day
  • Christmas Day
  • New Years Day

VACATION

Paid vacation time is available (at a pro-rated amount) on January 1 for all regular full-time employees. All employees would then receive the full amount they are eligible for on each January 1 thereafter.

1-5 years of employment – 1 week (2 weeks for Managers/Professionals)
5-10 years of employment – 2 weeks
